X-Ray Facility Registrants A. Yes, please call 217-785-9974 for assistance. Q. How do I register? A. Registration forms are available on our website. Completed forms may be submitted to the address on the form, or faxed to the Department. If you have any questions or need assistance, call 217-785-9974. Q. How much does it cost? A. Registration fees vary according to the type of installation and x-ray equipment, as noted in the regulations, Section 320.10(d). The fees vary from $35 to $170 per machine per year, and are billed in January. Q. How do I get inspected and how often? A. Inspection frequencies also vary by facility type, and may be annual, bi-annual, or on a 5 year cycle. Section 320.10(c) shows facility types and inspection frequencies. Q. How do I register as a diagnostic or therapeutic physicist? A. Forms and qualification criteria are found at http://www.illinois.gov/iema/radiation/pdf/appdistrp.pdf Q. When am I due for inspection? A. Have your registration number available and call 217-785-9974. Q. How much lead do I need for a new x-ray room? A. Lead shielding for x-ray rooms must be evaluated on a case by case basis. What's this fee for and why do I have to pay it? A. Effective January 1, 2000, fees for registration and inspection of x-ray equipment were combined into a revenue-neutral single annual fee, which is based on the facility and equipment type, and is billed each January.
